29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: On or about September 19, 2017, at the worksite located at 15500 Kanis Road in Little Rock, Arkansas, south side of Building 3, supported scaffold on the 3rd story balcony area, employees worked from a single 2 x 12 board platform to install porch header beam on the upper 4th story balcony. Employees were exposed to unprotected falls of greater than 25 feet to lower level.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: On or about September 19, 2017, at the worksite located at 15500 Kanis Road in Little Rock, Arkansas, south side of Building 3, supported scaffold on the 3rd story balcony area, guardrails were not installed on all open sides and ends to protect employees working from the single 2 x 12 board platform installing porch header beam on the upper 4th story balcony. Employees were exposed to unprotected falls of more than 25 feet to lower level. Or, in the Alternative: 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about September 19, 2017, at the worksite located at 15500 Kanis Road in Little Rock, Arkansas, south side of Building 3, the employer did not ensure employees on the 3rd story balcony working from a single 2 x 12 board platform installing porch header beam on the upper 4th story balcony, were protected from falling more than 25 feet to lower level.
