29 CFR 1926.552(c)(9): Safeties were not capable of stopping and holding the car and rated load when traveling at governor tripping speed.  1001 Julia St, New Orleans, LA:  On or about July 28, 2017, on bucket hoist #1, serial #20170302, the safety stop switch failed to stop the descent of the car exposing the employees to the hazard of a fall from the 8th floor to ground level.
