29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:    1409 Upland St., Metairie, LA:  On or about August 16, 2017, the employer allowed employees to utilize an electrical extension cord that was underneath standing water in a trench.  The extension cord was energized by a Honda EU2000i portable generator and was used to power a blower which circulated air inside of the trench.
