OSH ACT of 1970 Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall and struck-by hazards:     On or about September 27, 2017, employees were performing work on the communications tower when during the hoisting of loads by the attached gin pole, the rigging attachments failed, exposing employees who were tied off to the gin pole to fall hazards and employees on the ground, to struck-by hazards.
