29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely:     a) Worksite, Warehouse/Production Floor: Employees authorized to operate forklifts, including but not limited to, a Caterpillar and Nissan forklift, had not received training and evaluation in the safe operation of powered industrial trucks in over at least three years; on or about 9/26/17.     Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    a) Worksite, Warehouse/Production Floor, East Wall: Electrical panels were blocked by equipment such as but not limited to an Emmegi dust extractor; on or about 9/26/17.    b) Worksite, Warehouse/Production Floor, South Entrance: Electrical panels were blocked by equipment such as but not limited to a stepladder and pallet; on or about 9/26/17.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
