29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection: a) 1800 Arch Street, Philadelphia, PA 19103 - The employer did not install a complete guardrail system, in that top rails were missing in the system, on a walking/working level of a crane tower that was approximately 192 feet above the lower level, thus exposing employees to a fall from height hazard. This condition was observed on September 14, 2017.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) 1800 Arch Street, Philadelphia, PA 19103 - The employer did not install a stairway, ladder or other means for employees to access a bridge pick where there was a break in elevation of a approximately 27 inches to the pick from the lower level, the concrete floor. This condition was observed on September 14, 2017.
