OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to rack collapse and overhead struck-by hazards while working and walking in the warehouses: Instances 1 through 6: Location - 333 Fiske Street, Holliston, MA warehouse On or about September 27, 2017, the employer failed to anchor all warehouse racking uprights to the concrete floor in the following areas, such as but not limited to; Instance 1: Racking with workstations underneath the rack, Instances 2 through 6: Racking 04-05-02 to the back wall Instances 7 through 11: Location: 74 Lowland Street, Holliston, MA, warehouse On or about September 27, 2017, the employer failed to anchor at least five different sections of racking to the concrete floor in the following areas, such as but not limited to; Instance 7: Racking 34-05-02 Instance 8: Racking 33-05-02 Instance 9: Racking 32-05-02 Instance 10: Racking 11-15-02 Instance 11: Racking where white and blue rolling tables are stored.
