29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) was not protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board;    a.) 50 Budd St., Morristown, NJ.       An employee without any means of fall protection was exposed to an approximate 11ft fall hazard while engaged in painting activities from an extension ladder that was being utilized as a chicken ladder on the roof of a residential home,   on or about 10/03/2017.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a.) 50 Budd St., Morristown, NJ    Employees were exposed to fall hazards approximately 11 feet to the ground below as they ascended and descended a roof of a residential home when utilizing an extension ladder that only extended approximately 18 inches above the upper landing surface, on or about 10/03/2017.
