29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On July 12, 2017 employees performed elevator installation activities that included removing and re-installing bolts and washers from the lift brackets/platen or "dog ears". The task was apparently completed previously during the same day. Employees did not recognize the hazard of not performing the critical task of positioning the elevator sling assembly down on the buffer spring before removing the dog ear bolts. The employees did not recognize that the elevator sling assembly would descend when the bolts were removed. b) On July 12, 2017 an employee was exposed to a 9.5 foot fall hazard when working on top of the crosshead of the elevator sling assembly without a fall protection system in place. Employees did not acknowledge or recognize the fall hazard and take action to minimize of eliminate the hazard by performing the task from a properly positioned ladder. Employees did not use an apparently provided ladder to complete the task without straddling the crosshead. Note) Certification of corrective action is required.

29 CFR 1904.39(a)(2): The employer did not report to OSHA within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye resulting from a work-related incident: a) On July 12, 2017 an employee was hospitalized for three days as a result of a work place injury. The employer did not make the necessary notification of the incident to OSHA. Note) Certification of corrective action is required for this item.
