29 CFR 1926.451(c)(2):   Supported scaffold poles, legs, posts, frames, and uprights were not on base plates and mud sills or other adequate firm foundation.  On 10/02/2017, employees from O Donnell Exteriors were installing siding on a residential building located at 232 & 234 Pierce View Circle in Wentzville, MO.  The foreman was working from pump jack scaffolding at the rear of the residences.  Two of the five poles for the scaffolding were missing mud sills and or base plates or other adequate firm foundation.

29 CFR 1926.451(c)(2)(i):   Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement.  On 10/02/2017, employees from O Donnell Exteriors were installing siding on a residential building located at 232 & 234 Pierce View Circle in Wentzville, MO.  The foreman was working from pump jack scaffolding at the rear of the residences.  Two of the five poles for the scaffolding were not on a level surface and missing mud sills and or base plates.

On 10/02/2017, employees from O Donnell Exteriors were installing siding on a residential building located at 232 & 234 Pierce View Circle in Wentzville, MO.  The foreman was working from pump jack scaffolding at the rear of the residences.  The poles for the scaffolding were not secured to the structure by rigid triangular bracing or equivalent at the bottom.
