29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices;  a) On or about 10/05/17 in the saw area; an employee was exposed to laceration hazard while changing the blade on the horizontal band saw when the training provided by the employer did not prevent an employee other than the one performing the blade change applied their lock to the energy isolation device, i.e. disconnect.    b) On or about 10/05/17 in the saw area; employee was exposed to a laceration hazard while changing the blade on the vertical band saw when the training provided by the employer did not prevent an employee other than the one performing the blade change applied their lock to the energy isolation device, i.e. disconnect. 29 CFR 1910.303(a): The equipment required or permitted by this subpart were not approved:  a) On or about 10/05/17 throughout the shop; employees were exposed to contact with energized parts when the employer installed 4 square metal boxes for use as pendants drop outlets at or near each lather and mill location, such as but not limited to, NH Mill 4000, which had a knockout on the top of the metal box pushed in.        ABATEMENT CERTIFICATION REQUIRED
