Lockout devices shall be substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools. On or about October 5, 2017, the employer did not ensure the lockout device, known as a winder fork, used to isolate the electrical power from the drive chain and sprockets of the Line 10 winders was substantial enough to prevent removal while line operators were conducting setup and changeover of the Line 10 winder machines.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about October 5, 2017, the employer did not ensure their written energy control procedure for conducting setup and changeover of the winder machines covered all of the elements for the application of hazardous energy control. The procedure did not ensure energy isolating device operated in such a manner as to isolate the equipment from the energy source (electrical power) [per the 1910.147(d)(3) requirements], employees affixed a lockout device to the electrical energy isolation device [(d)(4)], and verified the isolation and deenergization of the machine had been accomplished [(d)(6)].

On or about October 5, 2017, the 110/120 volt flexible cord powering the bag sealer machine was not protected from accidental damage by sharp corners of packaging material and side of the machine or other pinch points. The outer covering (insulation) of the flexible cable was broken exposing the live conductors inside.
