29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    On or about October 5, 2017, the employer did not provide employees training to ensure adequate understanding of energy control procedures to safely access the Greenhouse Rewind feed side roller section for servicing, including inspection and adjustments (removing folds from the material). Employees were exposed to unexpected start-up likely to result in fractures and lacerations while performing servicing tasks.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1910.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Greenhouse Rewind machine - On or about September 29, 2017 through October 5, 2017, the employer did not provide guarding to protect employees working at or near areas where the plastic web ran over, around and between rotating rollers where ingoing nip point(s) and rotating part(s) exposed employees to caught-in hazards.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
