29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  (a)  Theatre - On or about October 5, 2017 two employees were exposed to a fall hazard while standing on the rails of two separate scissors lifts 22 feet above concrete with no fall protection.

29 CFR 1926.451(g)(1)(iv): Each employee on a self-contained adjustable scaffold shall be protected by a guardrail system (with minimum 200 pound toprail capacity) when the platform is supported by the frame structure, and by both a personal fall arrest system and a guardrail system (with minimum 200 pound toprail capacity) when the platform is supported by ropes;  (a)  Theatre - On or about October 5, 2017 two employees were exposed to a fall hazard while standing on the rails of two separate scissors lifts 22 feet above concrete with no fall protection.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:  (a)   Fountain Construction Co., Inc. - On or about October 5, 2017 employees were not trained on how to minimize the hazards associated with motorized scaffolds such as:  scissors lifts.
