29 CFR 1926.1053(b)(14): Cross-bracing on the rear section of stepladders shall not be used for climbing unless the ladders are designed and provided with steps for climbing on both front and rear sections. Tito Installer Co. Inc. project site located on Playbird Rd. Sheboygan, WI: On October 4, 2017, an employee was exposed to falls while straddling the head of a 10' step ladder, with one foot on the top step and the other foot on the rear brace.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.    Tito Installer Co. Inc. project site located on Playbird Rd. Sheboygan, WI: On and about October 4, 2017, employees were exposed to falls while using step ladders when they had not been trained to recognize and avoid fall hazards or to properly select and use ladders and had not been trained to understand and follow the OSHA standards that applied to their scope of work.
