29 CFR 1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.    Site: Corner of West 43rd Street & Broadway New York, NY    On or about 10/6/17    a) Employer did not provide training to an employee exposed to a fall hazard.  An employee was injured when they fell approximately 12 while working from a truss installing a banner.

29 CFR 1910.30(a)(3)(iv): The employer did not trained each employee on the correct use of personal fall protection systems and equipment, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer.  Site: Corner of West 43rd Street & Broadway New York, NY   On or about 10/6/17  a) An employee was not trained on the correct use of fall protection equipment.  An employee harness was not correctly hooked/attached and fell approximately 12 feet while installing a banner.
