29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  TMP America, Inc., Alpharetta, GA - Machine operators were exposed to an amputation hazard from openings on the stretcher that created a pinch point. The opening on the stretcher was located 44.5 inches from the floor on the side the employees walk-by, on or 10/11/17.

The end shaft was located approximately six feet from the floor, was three and one half inches in diameter and projected three inches out, on or 10/11/17.
