29 CFR 1926.502(f)(1)(iii): Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines:  On or about October 11, 2017, employees were exposed to a fall hazard of approximately 19 feet while accessing the roof and warning lines or other suitable fall protection was not used to connect the work to the edge of the roof.

29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:      On or about October 11, 2017, employee was exposed to a fall hazard approximately 19 feet while installing roofing material and a suitable fall protection system was not used.       Duerson, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard: 29 CFR 1926.501(B)(10)which was contained in OSHA inspection number 924396,citation number 1,  item number  1, AND WAS AFFIRMED AS A FINAL ORDER ON October 31, 2013  with respect to a workplace located at 105 Plank Road, Sycamore, Illinois, 60178.
