29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    a. World Class Plastics, Inc. - Russells Point, Ohio: On or about October 11, 2017, the employer did not ensure employees who setup and/or maintain the injection molding presses were adequately trained in the skills required for the safe application, usage, and removal of energy controls.  Employees that performed servicing tasks such as, but not limited to, setting molds and cleaning the mold were not trained to isolate and/or render energy sources safe.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a. World Class Plastics, Inc. - Russells Point, Ohio: On or about October 11, 2017, employees were exposed to crushing and struck-by injuries in that the #40 Sumitomo (s/n ARO174) injection molding press was not de-energized prior to servicing tasks such as, but not limited to, cleaning grease which was leaking from the mold.  The employer did not ensure that energy control application steps were implemented to control hazardous electrical/mechanical  energy of the machine's molds, and associated picking machine, prior to the servicing task; the employer did not:  -  Ensure that equipment was turned off or shut down.  -  Physically operate energy isolating devices to control the energy source(s).  -  Verify that isolation and de-energization of the machine or equipment had been accomplished.    b. World Class Plastics, Inc. - Russells Point, Ohio: On or about October 11, 2017, employees were exposed to crushing and struck-by injuries in that the #42 Sumitomo injection molding press and associated picking machine was not de-energized prior to servicing tasks such as, but not limited to, changeovers. World Class Plastics, Inc. - Russells Point, Ohio: On or about October 11, 2017, the employer did not ensure that the Yushin robot picker associated with press #42 was adequately guarded.  Employees working at the press to access parts on the conveyor or to empty the scraps bin were exposed to struck-by hazards.
