29 CFR 1926.451(e)(6)(vi): 29 CFR 1926.451(e)(6)(vi): Integral prefabricated scaffold access frames did not have a maximum spacing between rungs of 16 3/4 inches (43 cm). Non-uniform rung spacing caused by joining end frames together is allowed, provided the spacing does not exceed 16 3/4 inches (43 cm). The employer did not ensure that employee access to the scaffold had a maximum rung spacing of 16 3/4 inches. The employees were accessing the scaffold by using the scaffold rungs that were measured at approximately twenty (20) inches. This resulted in the employees being exposed to a fall hazard of approximately twenty (20) feet to the ground.

29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section did not comply with the provisions listed in subparagraphs (i) through (xv) of this paragraph. The employer did not ensure that guardrails were installed on the open side of the scaffold when employees were using it as work surface. This resulted in the employees being exposed to a fall hazard of approximately twenty (20) feet to the ground.
