29 CFR 1926.501(b)(13):  Each employer engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented:     The employer is failing to ensure employees are protected from fall hazards when conducting installation of shingles as part of re-roofing operations.  Workers were observed installing shingles on a residential style construction site (house) without utilizing fall protection.  Some of the employees on the roof were wearing harnesses but not tied off to anything  and employees were on the roof without any form of fall protection being utilized.  The workers were exposed to falls of over 10 feet to dirt and concrete below.  This was most recently documented at 11604 Eastlake Circle, Papillion, Nebraska on or about October 12th, 2017 when the employer had employees working on the roof of this residence without any form of protective system.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      The employer is failing to provide employees with adequate and effective fall protection training prior to allowing them to work on a residential style roof. Workers were observed installing shingles on a residential style construction site (house) without utilizing fall protection.  This training would allow each employee to recognize the hazards of falling and allow them to understand the proper procedures to be followed in order to minimize these hazards.  The workers were exposed to falls of over 10 feet to dirt and concrete below.  This was most recently documented at 11604 Eastlake Circle, Papillion, Nebraska on or about October 12th, 2017 when the employer had employees working on the roof of this residence without any form of protective system and had not provided any training to them.

29 CFR 1926.1053(b)(1):  Were portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     The employer is failing to ensure employees are protected from fall hazards while utilizing portable ladders as part of re-roofing operations.  Workers were observed climbing up a portable extension ladder which did not meet the elements of this standard.  Instances in which the ladder was in violation of this standard are as follows:                  a)  The ladder did not stand three feet above the upper landing surface (roof) of the building.     b)  The ladder was not tied off at the top to preclude the ladder from sliding and falling as the employees utilize it.   The workers were exposed to falls of over 18 feet to dirt and concrete below.  This was most recently documented at 11604 Eastlake Circle, Papillion, Nebraska on or about October 12th, 2017 when the employer had employees working on the roof of this residence with the above listed issues present as they were going up and down the ladder on site.

29 CFR 1926.1060(a):  The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:     Employees had not been trained on the proper use of portable ladders.  Multiple hazards involving the set-up and usage of the ladder on site were identified during the inspection.  These hazards and the lack of training exposed the employees to a fall hazards in excess of 9 feet to the dirt and concrete on this site.     The employer is failing to provide employees with adequate and effective ladder safety training prior to allowing them to work on a residential style roof.  This training would allow each employee to recognize the hazards of falling and allow them to understand the proper procedures to be followed in order to minimize these hazards.  Workers were observed climbing up portable extension ladders which were set up incorrectly.  Had the employees had this training the following observed instances would not have been present on the job site.  Instances in which the ladder was in violation of this standard are as follows:         a)  The ladder did not stand three feet above the upper landing surface (roof) of the building.        b)  The ladder was not tied off at the top to preclude the ladder from sliding and falling as the   employees utilize it.   The workers were exposed to falls of over 18 feet to dirt and concrete below.  This was most recently documented at 11604 Eastlake Circle, Papillion, Nebraska on or about October 12th, 2017 when the employer had employees working on the roof of this residence with the above listed issues present as they were going up and down the ladder on site.
