On or about October 1, 2017, employee(s) who stood partially underneath a vehicle elevated on the hydraulic forks of the FiatAllis loader in order to remove an axle shaft were exposed to struck-by hazards. 29 CFR 1910.244(a)(1)(i): The operator did not ensure that the jack used had a rating sufficient to lift and sustain the load:  Outside Shop Area.  On or about October 1, 2017, employee(s) selected and placed a single jack as auxiliary protection beneath a vehicle elevated on the hydraulic forks of the FiatAllis loader.  The jack, rated at approximately 3,000 lbs., was not adequate to lift and sustain the vehicle load.  There is no abatement documentation or certification required for this item.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet:  Shop Area.  Oxygen and acetylene cylinders no longer in use were stored together, creating a potential fire hazard.  No abatement documentation or certification is required for this item.
