29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: Jobsite: The employees, working from the Alum-a-Pole fabricated platform/plank of the ladder jack scaffold were not protected from the hazard of falling approximately 11 feet to the ground by the use of appropriate fall protection systems/devices.

"When leaving a powered industrial truck unattended, load engaging means shall be fully lowered, controls shall be neutralized, power shut off, brakes set, key or connector plug removed": Jobsite: The All-Terrain Skyjack Telehandler forklift was unattended while being used to support the south end of the Alum-a-Pole fabricated scaffold plank from which employees were working.
