On or about October 12, 2017, an employee installing vinyl siding on the north side of a commercial building was exposed to fall hazards and a competent person did not conduct an inspection of the job site to identify and eliminate the hazards.

29 CFR 1926.453(b)(2)(v): The employer did not ensure a lanyard was attached to the boom or basket when working from an aerial lift: On or about October 12, 2017, an employee installing vinyl siding on the north side of a commercial building was exposed to fall hazards of approximately 3 stories while working from the basket of a JLG 600 AJ aerial lift and a lanyard was not attached to the basket or boom.
