29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) The Willoughby Office Building, East side: On or about 10/20/2017, and at times prior, the employer did not provide training to each employee performing work on an aerial lift, in that the employer failed to provide fall protection PPE needed while performing work within the aerial lift basket. The employee was not trained to recognize the unsafe conditions while using this equipment.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a) The Willoughby Office Building, East side: On or about 10/20/2017, and at times prior, the employer did not provide training to each employee performing work on an aerial lift, in that the employer failed to provide fall protection PPE needed while performing work within the aerial lift basket. The employee was not trained to recognize the unsafe conditions while using this equipment.
