29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross-braces were used as a means of access.  a) 1123 Reinoehl - The employer did not ensure employees utilized ladders to gain access to the scaffold platform, allowing employees to use the 2 x 4 bracing to climb to reach the scaffold platform.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:  a) 1123 Reinoehl St - The employer did not ensure that employees working on a scaffold approximately 11 ft to the ground below were protected from falling by personal fall arrest, guardrail or safety net systems, thereby exposing employees to fall hazards on or before October 20, 2017.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.452(k)(2): The support used for ladder jack scaffolds did not meet the requirements of Subpart X - stairways and ladders.  a) 1123 Reinoehl St - Employees worked from a scaffold platform that was supported on ladder jack scaffold brackets. the top of the ladder jack bracket was attached to a wooden 2 x 4 while the bottom bracket was not supported by anything, thereby exposing employees to fall hazards of approximately 11 feet to the ground below on or before October 20, 2017.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  OR IN THE ALTERNITIVE:  29 CFR 1926.451(b)(2): Scaffold platforms or walkways were not at least 18 inches wide:   a) 1123 Reinoehl St -  Employer did not ensure that employees which were climbing 2 x 4's  on the west side of the structure to work and gain access to the ladder jack scaffold located on the south side were protected from falling at heights 6 feet or higher, thereby exposing employees to fall hazards on or before 10/20/2017.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
