29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: Location - production area On or about October 20, 2017, the employer did not ensure that drenching facilities were properly maintained. 1. West side - water flow from eyewash was not sufficiently high to allow employees use. 2. East side - eyewash was blocked by obstructions.

29 CFR 1910.1026(c): Employees were exposed to an airborne concentration of chromium (VI) which exceeded 5 micrograms per cubic meter of air, as an 8-hour time-weighted average: Location - production area On or about October 31, 2017, the employer did not ensure that an employee's exposure to hexavalent chromium does not exceed 5 microgram per cubic meter of air, as an eight-hour time-weighted average. The employee who worked with 1700 gallon chrome tank was exposed to hexavalent chromium at an eight-hour time-weighted average of 6.8ug/m3 of air, approximately 1.36 times the OSHA PEL of 5.0ug/m3.

An employee ate lunch nearby eyewash station (east side) at the worksite where hexavalent chromium is present. 2. An employee ate lunch while working on chrome plating at worksite where hexavalent chromium is present.
