29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:       a) Entire pole barn, on or about 10-20-17:  Employees were accessing work platforms by climbing on the building framing, exposing the employees to a fall hazard of up to 16 feet.

29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:       a) Entire building, on or about 10-20-17:  Employees were installing purlins to the top of roof trusses while standing on the trusses and previously installed purlins, without fall protection, exposing the employees to a fall hazard of 16 feet up to 25 feet.
