29 CFR 1926.453(b)(2)(v): The employer did not ensure that employees wore a body belt with lanyard attached to the boom or basket when working from an aerial lift.  This violation occurred on October 24, 2017, at a jobsite located at 3500 McCann, Longview, Texas  75605, where at least two (2) employees worked on installing banners on light poles while working in an aerial lift without the use of body belts and lanyards.
