29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501:    (a) JCT Roofing LLC at 1721 East 68th Avenue, Denver, CO 80229: On or before 10/23/17, the employer did not insure that employees were protected from fall hazards while performing roofing work on a residential property. Three employees were tearing shingles off of a roof without the use of fall protection or a fall protection system. This condition exposed the employees to fall hazards of approximately 8 feet.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:     (a) JCT Roofing LLC at 1721 East 68th Avenue, Denver, CO 80229: On or before 10/23/17, the employer did not insure that employees were properly using a portable ladder for access to the rooftop of a residential property. The employees were accessing the roof to remove old shingles and the ladder did not extend at least 3 feet above the surface being accessed nor was the ladder secured to a rigid support. This condition exposed employees to fall hazards from approximately 8 feet.
