Signs affixed to the doors were not illuminated by emergency lighting within the building.

Clark #4 Model C500 50 LPG, Serial Number 355 842 2528 warning labels had been worn and were illegible.  2. Toyota forklift did not have a data plate to indicate the Model, Serial Number or the vehicle's rated capacity.  Warning labels on the vehicle had been damaged and were illegible.  3. Datsun 6000 data plate and warning labels had been damaged and were illegible.  4. Daewoo D355 data plate had been damaged and was illegible.

On or about October 25, 2017, Office, near rest room:  A metal storage cabinet and empty cardboard boxes were stored within the working space in front of a 120V electrical panel, preventing easy access for maintenance or inspection.  Labeling in the panel identified that it powered the second floor office lights, exit and emergency lights, truck lights, first and second floor air handler.

On or about October 25, 2017, Block Plant, at the back of the block making equipment:  A 31"-diameter metal encased MaxxAir floor fan was not grounded.  Stray current was detected in the metal parts of the fan, exposing employees to electric shock hazards.

On or about October 25, 2017, Block Plant, behind block making equipment:  The flexible cordset for the 31" diameter metal-encased floor fan had the insulation pulled from its fitting.  Strain on the cordset damaged the ground wire.
