29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   1) On or about 10/26/2017, at the site of 45-28 251st Street, Little Neck, NY  Employees installing shingle on a pitch roof with the ratio of 12 in 12 were exposed to the hazard of falling approximately 15 ft. to the ground below.   ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 ft (0.9 m) above the upper landing surface to which the ladder was used to gain access:    A)        on or about 10/26/17, on the site of 45-28 251st Street. Little Neck, NY    Employees were using a portable ladder for gain access to a roof. The ladder side rails extended one foot above the upper landing surface. The employees were exposed to the hazard of falling approximately 20 ft to concrete ground below.     ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
