29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:      a) Stern Pinball Inc. - On October 26, 2017, in the wire crimping line, the plexiglass used to guard the point of operation on all Modlex terminator crimp presses, was installed with 3 inch by 1 inch  opening which allowed operator access to the point of operation.

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:     a) Stern Pinball Inc. - On October 26, 2017, in the production department, the employer did not ensure that all surfaces were maintained as free as practicable of accumulation of lead. A wipe sample collected from the west side of the solder pot work table indicated the presence of 26.13 micrograms of lead.      b) Stern Pinball Inc. - On October 26, 2017, in the production department, the employer did not ensure that all surfaces were maintained as free as practicable of accumulation of lead. A wipe sample collected from the east side of the solder pot table indicated the presence of 7.54 micrograms of lead.

29 CFR 1910.1200(h)(2)(ii): The employer did not provide information to the employees on operations in their work area where hazardous chemicals were present.  a) Stern Pinball Inc. - On October 26, 2017, all employees who conducted soldering operations were not provided information and training on hazardous chemicals. The employees were exposed to hazardous chemicals, including but not limited to Allen C-208 Alcohol/Chloride Flux and 48 Lead Free Flux Cored Solder.
