29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a.  On or about October 26, 2017, at the above addressed jobsite, employees conducting roofing operations on steep sloped roof, greater than twenty-five (25) feet above the ground, were not protected from falling.   Alpha GC Inc., was previously cited for an equivalent Occupational Safety and Health Standard 29CFR 1926.501(b)(10), which was contained in OSHA inspection number 1049459, citation number 1, item number 3 and was affirmed as a final order on August 20, 2015, with respect to a workplace located at 4619 W. Roscoe Ave., Chicago, IL  60641.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a.  On or about October 26, 2017, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
