29 CFR 1926.451(b)(1)(i):   Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide and the employer could not demonstrate that a wider space was necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform).  On 10/24/2017, employees from Allen Roofing & Siding were installing siding on a residential building located on lot 29 Catherine Court in O'Fallon, MO.   An employee was installing siding on the front of the residence while standing on tubular welded frame scaffolding.  The platform the employee was working on was not fully planked.

29 CFR 1926.451(e)(1):   When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used.   On 10/24/2017, employees from Allen Roofing & Siding were installing siding on a residential building located on lot 29 Catherine Court in OFallon, MO.   An employee was installing siding on the front of the residence while standing on tubular welded frame scaffolding.  The first row of planking was 39-41 inches above the ground.  The employee did not use portable ladders, hook-on ladders, attachable ladders, ramps, walkways, or similar surface to access the scaffolding.

On 10/24/2017, employees from Allen Roofing & Siding were installing siding on a residential building located on lot 29 Catherine Court in OFallon, MO.   An employee was installing siding on the front of the residence while standing on tubular welded frame scaffolding.  The scaffolding had not been inspected for visible defects by a competent person.  The scaffolding was missing cross bracing and not fully planked at the working level.

The cross braces were not of such length as would automatically square and align vertical members so that the erected scaffold is always plumb, level, and square.   On 10/24/2017, employees from Allen Roofing & Siding were installing siding on a residential building located on lot 29 Catherine Court in OFallon, MO.   An employee was installing siding on the front of the residence while standing on tubular welded frame scaffolding.  The scaffolding was not adequately braced by cross, horizontal, or diagonal braces, or combination thereof, which secured vertical members together laterally.
