29 CFR 1926.552(c)(17)(iv)(a): Wire rope shall be taken out of service when any of the following conditions exist:  In running ropes, six randomly distributed broken wires in one lay or three broken wires in one strand in one lay:  On or about October 20, 2017, during the construction of an underground sewer relief tunnel the employer did not remove from service a wire running rope missing an entire rope strand.  The wire rope was wound on the drum of a hydraulic powered pull master winch and used to hoist material.

29 CFR 1926.553(a)(1): Exposed moving parts such as gears, projecting screws, setscrews, chain, cables, chain sprockets, and reciprocating or rotating parts, which constitute a hazard, shall be guarded.     On or about October 20, 2017, during the construction of an underground sewer relief tunnel the employer did not ensure the hazardous exposed moving and rotating cable (wire running rope) and drum of a hydraulic powered pull master base-mounted drum hoist (winch) was guarded.
