Site: 85 Harristown Rd., Glenrock, NJ.  a) Employees servicing electrical lighting did not lock out and tag out the equipment from its power source or stored energy; on or about 10/9/17.

29 CFR 1910.333(c)(2): The employer did not ensure that only qualified persons were working on energized circuits and that qualified persons were familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools.  Site: 85 Harristown Rd., Glenrock, NJ.   a) An employee servicing an electrical light fixture did not use safe procedures prior to working on the equipment he knew was energized; on or about 10/9/17.
