29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval:    For the employees working from the job made man basket attached to the forks of an all terrain material handler at 2401 West Walnut Road in Rogers Arkansas, on or about October 31, 2017 and times prior there to the employer constructed a wood basket and allowed employees to work from the basket attached to the forks of an all terrain material handler at heights greater than 6 feet, without the manufacturer's approval.
