29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about 10/31/17 located at 3875 Grandview Manor Dr., Cumming, GA: Employees were roofing the house that was under construction while on top of the roof approximately 35  foot from the lower level. The employee on the roof was throwing and dropping metal flashing to the ground and the employee below was without a hard hat. The boss nearly hit his employee in the head with a 4 x 4 x 6 metal flashing near miss of being struck in the head without hard hats exposing them to a struck by hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about 10/31/17 located at 3875 Grandview Manor Dr., Cumming, GA: Employees were observed and exposed to fall hazards, while roofing a house that was under construction and the employees were using a portable extension ladder to gain access to the roof, and the ladder was only 12- inches above the landing surface on the 2nd roof.
