On or about November 02, 2017 in the Laundry Department, employees were exposed to the four inclined drive belts of the Open Pocket E washing machine.  The end cap of the guard had been removed and was lying behind the washing machine.  b. On or about January 05, 2018 in the Laundry Department, employees were exposed to the four inclined drive belts of the Open Pocket A washing machine.  The end cap of the guard had been removed and was lying behind the washing machine.

The junction box's door was sprung which prevented it from remaining closed.
