29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) Joe's Crab Shack Restaurant, Back side, On or about 11/01/2017, and at times prior, the employer exposed employees to struck-by injuries while working in a 5-feet, 7-inch deep excavation, in that head protection was not provided to employees performing plumbing working within the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) Joe's Crab Shack Restaurant, Back side: On or about 11/01/2017, and at times prior, the employer exposed employees to cave-in hazards while working in an excavation which measured 33-feet long by 10-feet wide by 5-feet, 7-inches deep, in that a cave-in protection system was not provided to employees engaged in new grease trap and sewer line installations.
