29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.    On or about November 2, 2017, employees were exposed to a fall of approximately 17 feet.  The employees were conducting steel erection activities on a new commercial building located at 1020 West State, Salem, Ohio 44460. The employees were using an articulating lift to access the roof without wearing a personal fall arrest system.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  On or about November 2, 2017, employees were exposed to a fall of approximately 17 feet.  The employees were conducting steel erection activities on a new commercial building located at 1020 West State, Salem, Ohio 44460. The employees were working on all areas of the roof without using any form of fall protection.
