29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another:     a) On or about 11/02/17, on a roofing jobsite at a house on 55 Moselle St., Buffalo, NY:  Employees were  exposed  to a potential fall of approx. 17 ft. from the aluminum piks on a Ladder Jack Scaffold because the piks were overlapped (bridged).     NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) On or about 11/02/17, on a roofing jobsite at a house on 55 Moselle St., Buffalo, NY:  Two employees working on the ground doing clean-up work in the vicinity of employees working  above on a Ladder Jack Scaffold  were exposed to the potential hazard of being struck by materials  that could fall off the scaffold.  The employees were not wearing a hardhats.      NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about 11/02/17, on a roofing jobsite at a house on 55 Moselle St., Buffalo, NY:  Employees  used  an extension ladder to access the "pik" of  a ladder jack scaffold. The siderails of that extension ladder extended  approx.  30 inches above the edge of the pik.     NO ABATEMENT CERTIFICATION REQUIRED
