Work areas to include walk ways and around machines were cluttered with printing operations debris which included items such as boxes, plastic containers, cords, a mobile storage rack and a mobile compressor. Portable fire extinguishers were located on the floor behind a rag can and among other garbage that had not been discarded. 29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative: a) Facility: On November 2, 2017 and at times prior thereto employees performed scheduled and unscheduled maintenance on machines including but not limited to the Heidelberg 728 printing press and Heidelberg Polar-Mohr cutter. Specific, written energy control procedures were not developed by the employer to be utilized by authorized employees when performing scheduled and unscheduled maintenance. Training had not been provided to authorized and affected employees to identify their role and responsibility in an energy control program. Periodic inspections were not conducted to ensure employees performing scheduled and unscheduled maintenance were adequately protected from hazardous energy sources. 29 CFR 1910.212(a)(1): One or more method of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Facility: On November 2, 2017 and at times prior thereto a full guard was not in place for the flywheel on the Heidelberg Windmill. The flywheel was low to the ground, exposing employees in the area to a caught by hazard. b) Facility: On November 2, 2017 and at times prior thereto an adjustable guard was not in place on the blade of the Bridgewood band saw, exposing the operator to an amputation hazard. c) Facility: On November 2, 2017 and at times prior thereto a guard was not in place for the belt and pulley on a flywheel on the pre-press compressor, exposing employees to a caught-by hazard. 29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met: a) Facility: On November 2, 2017 and at times prior thereto the employer had not implemented and maintained a hazard communication program which would provide employees with basic information about health effects, precautions and personal protective equipment needed for the chemicals used in the work place. Employees regularly used hazardous chemicals, which include but are not limited to GANS MRC metering roller cleaner, low odor wash and isopropyl alcohol, for printing and cleaning of various printing press machines. A comprehensive list of hazardous chemicals used in the workplace was not available upon request. Cleaning chemicals were stored in unlabeled containers.
