29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a) Jobsite, 401 Richland Avenue, Wheeling, West Virginia:  On or about November 3, 2017, two employees were operating a nail gun to install shingles on a residential roof and were not wearing eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     a) Jobsite, 401 Richland Avenue, Wheeling, West Virginia:  On or about November 3, 2107, two employees were installing shingles on a residential roof, approximately 35-40 feet above the ground without utilizing fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) jobsite, 401 Richland Avenue, Wheeling, West Virginia: On or about November 3, 2017, the employer did not have a training program for the employees who routinely work at heights and were working on a residential roof at heights of 35-40 feet, without fall protection.
