The condition was most recently observed at 825 South Lindbergh Boulevard, St. Louis, Missouri, when High-Reach (Order-Picker) Forklift operators are exposed to an approximate 15 foot fall hazard while operating the Caterpillar Order-Picker, m/n NOR30P without fall protection.
