29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a)  Grinding Area - On or about November 7, 2017 and at times time prior, employees were exposed to amputation hazards in that the blaster discharge #E2 was not guarded and was located at a height of less than 6 feet exposing employees to amputation hazards.

29 CFR 1910.133(a)(3): The employer did not ensure that employees wearing prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design or wears eye protection that can be worn over the prescription lenses.  a)  The production area - On or about November 7, 2017 and at times prior,  at least one employee normally wears prescription glasses and the employer did not provide prescription safety glasses or safety glasses that would be worn over his prescription glasses.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a) Production Area - On or about November 7, 2017 and at times prior the employer provided employees with two strap N95 disposable respirators to be used on a volunteer basis and has not provided employees with the information contained in Appendix D of the respiratory protection standard.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  a)  The diesel tank behind the production area - On November 7 and at times prior, employees were exposed to chemical hazards in that the tank had no marking to indicate it to be a combustible liquid or its contents.
