29 CFR 1910.178(l)(3)(i)(G): Powered industrial truck operators were not trained in topics applicable to the safe-operation of the vehicles, specifically, fork and attachment adaptation, operation, and use limitations.      a) Production Area - On or about October 30, 2017, the Caldwell Lift-Truck, Model Number FB 40, Serial Number 06-70971, lifting attachment was secured to a forklift and used to lift and transport rolls of plastic film onto Blanking Machine Number 1.  The hook at the tip of the attachment was missing the manufactured hook latch.    Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the guidelines in the Caldwell Lif-Truc, Fork Truck Boom Attachments, Instruction Manual for inspections of the equipment.
