29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  Dieterich Standard, Inc. at 5601 N. 71st St., Boulder, CO 80301: On and before November 8, 2017, the employer did not ensure that one or more methods of machine guarding was provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks, in the following instances:      1)  The Colchester Clausing Model 600 18", serial number MTO191 was not adequately guarded and exposed employees in the area to hazards from ingoing nip points and rotating parts;       2)  The HYDMECH Model H-14A horizontal band saw included a non-working part of the blade that was not guarded;      3)  The Okuma L470CNC had an opening on the back side approximately 6 inches in diameter for a bar feeder that was not guarded.  In addition, the employer did not provide a table to support the bars to prevent the bars (materials) from falling on the operator, and to prevent employees in this area from being impacted by the bars; and      4)  The Okuma Multus B300W had an opening on the back side approximately 6 inches in diameter for a bar feeder that was not guarded.  In addition, the employer did not provide a table to support the bars to prevent the bars (materials) from falling on the operator, and to prevent employees in this area from being impacted by the bars.  These conditions exposed employees using this equipment to amputation, laceration, ingoing nip points, and rotating parts hazards.
