29 CFR 1910.178(m)(2): No person shall be allowed to stand or pass under the elevated portion of any truck, whether loaded or empty. a)On 11/9/2017 an employee was in the process of reattaching a scale to a forklift that had been knocked loose during operations. The employee utilizing a second forklift to support the scale, while loosening the bolts holding the scale in place, the employee's left hand was under the attachment, the scale fell resulting in the employee's left index finger being caught under the scale.

Caution shall be exercised when handling off-center loads which cannot be centered. a)On 11/9/2017 an employee was in the process of reattaching a scale to a forklift that had been knocked loose during operations.
